SDGA Tournament Entry Form
All entries must be received at the NYSGA Offices by no later than 5PM on the Entry Close Date. Any entry received after the Entry Close Date will not be accepted, regardless of circumstances relating to submission of a late entry.

Incomplete entries will be returned to the applicant. All entries are subject to approval by the SDGA Committee which reserves the right to reject an entry for any reason deemed sufficient by it.
